For old ferrets, it would be very dangerous to operate and especially the liver, I personally would not do surgery, but let him live out his life in happiness. The splein is part of the immune system and can swell from an infection, or possibly he may have tumors there also, you might want to try some antibiotics抗生素 to see if the splein will shrink.
